Photography Authorisation Form
During the tournament, the official tournament photographer will take photographs and video images, which may be used for publicity purposes or in promotional material by the Tournament Office.
In line with legislation recently introduced by the British Government, the Tournament Office requires advance permission for any photography and video images to be used by the Tournament Office for publicity or promotional purposes.
It is essential that you review this form giving your consent as Team Manager/Secretary acting in ‘loco parentis’ (in place of parents) for photography and video images taken by the official tournament photographer to be used for publicity or promotional purposes.
Please provide the Tournament Office with the names of any individual child who does not wish to be photographed at this Tournament.
